What You Need for an Arrangement Conference

Things to Expect In An Arrangement ConferenceAt the arrangement conference we will need to obtain legal and vital information for completing the death certificate and information for finalizing the burial or cremation. Please bring the following items:

• Drivers License or other photo I.D., such as passport
• Social Security Card
• Military Discharge Papers (Form DD214)
• Recent Photograph for preparation purposes and for newspaper obituary notice or custom memorial folders
• Clothing-all normal items including undergarments. For women, an outfit closed at the neckline and long sleeves. Shoes are not required.
• Please allow 2 hours or more for the Arrangement Conference.
• Death certificates are prepared by the funeral home, signed by the physician, coroner or medical examiner and then filed by the funeral home with the Office of Vital Records. Certified copies of the Death Certificate may be necessary for the following:
• Probate of Will
• Final Tax Return
• Stocks and Bonds
• Life Insurance
• Bank accounts
• Pension, IRA, or other retirement benefits
• Motor Vehicles
• Real Property
• Personal Record of Death
